Complete list of documents required for a successful APA application

Preparing an APA file requires gathering specific documents, and a single omission can delay processing by several weeks. The departmental council only initiates the evaluation of loss of autonomy after receiving a complete file. It is therefore better to know each expected document before filling out the form.

Cerfa or departmental form: identify the correct APA application support

Not all departments offer the same starting document. Some use the national cerfa form (cerfa DAA), while others have created their own version. First and foremost, check your departmental council’s website to see which one is in effect.

Since 2024, several departments are testing a shared APA, ASH, and mobility inclusion card file. A single form then replaces three separate procedures. The departments of Nord and Isère, for example, have set up this common platform. If you fall under one of these departments, you will only need to provide one proof of income and residence for all aids.

Digitalization is also progressing. While handwritten signatures on paper remain the norm in most cases, some departments now accept a digital signature via FranceConnect, accompanied by a scanned copy of the identity document. Inquire: submitting a file online can save several days of processing time.

To clearly identify all the documents required for an APA application, it is useful to distinguish between identity documents, those related to income, and those related to medical situation.

Identity documents and proof of residence for the APA file

The first reflex is to gather the documents that prove who you are and where you live. These documents determine the admissibility of the file.

  • A copy of the identity card, passport, or residence permit that is currently valid. For non-European Union nationals, the residence permit must cover the period of the application.
  • A proof of residence less than three months old: utility bill, rent receipt, property tax notice. If the elderly person lives with a relative, a housing certificate signed by the host, accompanied by their own identity document, replaces the bill.
  • A bank account statement (RIB) in the name of the applicant, necessary for the payment of the allowance. If a guardian manages the finances, the RIB of the protected account is required.

Do you live in an institution or a foster family? Add a certificate from the institution specifying the date of entry, or the family hosting contract. These situations change the calculation of the APA, as the department distinguishes between home APA and institutional APA.

Proof of income: tax notice and beyond

The amount of the APA directly depends on the applicant’s income (and their spouse’s, if applicable). The most recent tax notice or non-tax notice is the central document. It is used to calculate the potential financial contribution of the beneficiary.

The department takes into account declared income, but also certain asset elements. If you receive supplementary retirement pensions, life annuities, or rental income, prepare the corresponding proofs.

Are you already receiving social assistance, housing allowance, or disability pension? Attach the notifications of these benefits. They allow the medico-social team to have a complete view of your situation without asking you for information that is already available.

Medical certificate and evaluation of loss of autonomy GIR

The medical aspect is often the one that raises the most questions. The APA file requires a recent medical certificate describing the health status and functional limitations of the applicant. This document is written by the attending physician.

The certificate does not need to mention a GIR classification. It is the departmental medico-social team that visits the home to assess the loss of autonomy using the AGGIR grid (Autonomy Gerontology Resource Groups). Only GIR 1 to 4 are eligible for the APA.

In practice, a detailed medical certificate speeds up the evaluator’s work. Mention to the doctor the concrete difficulties: bathing, dressing, moving, meal preparation. The more precise the description, the more accurate the evaluation will be to daily reality.

Main caregiver block: an increasingly requested document

Since the law of April 15, 2024, on supporting family caregivers, an increasing number of departments require information about the main caregiver as soon as the file is submitted. Identity, contact details, and relationship of the caregiver are now included in a mandatory block on certain revised forms.

Why this evolution? Because the aid plan increasingly incorporates the needs for respite and support for the caregiver. Providing this information from the start avoids a subsequent file supplement and allows the medico-social team to propose a more tailored aid plan during the home visit.

If the elderly person is already using a home help service, attach a copy of the contract or the latest invoice. This proof helps the department assess existing needs and adjust the aid plan accordingly.

A complete APA file consists of about ten documents, but each department may add specific documents. Before mailing or submitting your file, call the autonomy service of your departmental council to confirm the exact list. A five-minute phone call can prevent a three-week postal round trip.
